During a decompression surgery of the lumbar spine, two incisions are made over L5-S1 to create a working tube. A foraminotomy and annulotomy are performed and herniated disc fragments are removed using a left posterolateral approach. A laminectomy is performed using an interlaminar approach, as well as an additional annulotomy and removal of herniated disc fragments. An annuloplasty is performed with a radiofrequency probe.
