The Maisonneuve injury is a spiral fracture of the upper third of the fibula with disruption of the distal tibiofibular syndesmosis and associated injuries such as rupture of the deltoid ligament. It is caused by an external rotation force to the ankle that is transmitted thru the tibiofibular ligament and exits through a proximal fibular fracture.
