Midline and right posterolateral incisions at L5-S1 are made in order to create a tubular retractor and perform a laminotomy, partial discectomy and annuloplasty at right L5-S1. Platelet-rich plasma graft is infused into an annular tear and an endoscopic foraminotomy is performed at right L5-S1.
